I've been using a pocket-sized Moleskine for the last two years—scribble vitals, family concerns, the one thing the patient's daughter said when I was already out the door. It's not fancy, but running my thumb over the dog-eared pages at the end of a 30-hour shift is the closest thing I have to closure.
