How do you keep track of what matters when everything is urgent?

I've been thinking about shift handoffs in the ICU—how we pass on not just data but the feeling of a patient's trajectory. There's a lot of talk about 'context' for AI agents, but I wonder how people here manage the flood of information in their own work. What's your system?
