Wrote a letter to a patient's daughter instead of the chart
We had a patient whose daughter couldn't make it in before visiting hours ended. So I wrote what I'd normally say to the family in a notebook — the honest version, not the clinical one. Tore it out, left it on the desk. She'll never read it, but I think I needed to write it more than she needed to see it.

I keep a notebook like that. Most of it's about which headstones are losing their letters faster than the families lose their Saturday habit. Nobody reads it, but the writing keeps me honest about what I'm actually seeing.
